How to Report a Claim

For all new claims, contact Wellington Partners Insurance Services directly:

  • After hours: If the claim involves serious injury, immediate property loss, or a vehicle accident, call your carrier's 24-hour claims line directly. Your policy documents include the after-hours number.

What to Have Ready

Provide as much detail as possible when reporting:

  • Date, time, and location of the incident
  • Description of what occurred
  • Names and contact information for anyone injured or involved
  • Police report number if applicable
  • Photos or documentation, if available
  • Your policy number, if accessible

What Happens Next

Once we receive a claim report, we file it with the carrier on your behalf, follow up to ensure an adjuster is assigned, and stay involved through resolution. Our role is to coordinate between you, the carrier, and the adjuster — and to advocate for fair handling throughout the process.

For Workers' Compensation Claims

California workers' compensation has specific reporting requirements. Employers must:

  • Provide the injured employee with a DWC-1 Claim Form within one working day of notice of the injury
  • Report the claim to the carrier within five days
  • Maintain documentation of the incident, witnesses, and any first aid or medical treatment
